XER-001 is a reformulated version of the radioprotective drug amifostine that is being developed by Xerient Pharma for nasoduodenal delivery as an adjunct to stereotactic body radiotherapy in patients with locally advanced pancreatic adenocarcinoma.[5][7][11][12][13] Amifostine is a phosphorothioate prodrug that is dephosphorylated in normal tissues to an active free thiol metabolite, which scavenges reactive oxygen species and free radicals generated by ionizing radiation, thereby preferentially protecting healthy tissues from radiation-induced damage while sparing tumor tissue.[8][11] XER-001 is intended to repurpose this mechanism via targeted gastrointestinal administration to mitigate radiation injury and potentially improve the therapeutic index of high-dose pancreatic radiotherapy.
