**XG004** is an investigational topical small-molecule prodrug conjugate developed by Xgene Pharmaceutical for pain associated with knee osteoarthritis. It is a covalent naproxen-pregabalin conjugate designed to provide dual activity against nociceptive and neuropathic components of pain. Following topical administration as a gel, XG004 and its active metabolites, including naproxen and pregabalin, were detected in plasma and synovial fluid in a completed randomized placebo-controlled Phase 1b study in painful knee osteoarthritis. Naproxen-derived activity inhibits cyclooxygenase enzymes, while pregabalin-derived activity binds voltage-dependent calcium-channel alpha-2/delta subunits.
