Xiakucao oral liquid is a traditional Chinese medicine (TCM) preparation derived from the plant *Prunella vulgaris* L. (Self-heal). It is primarily used in China for the treatment of thyroid-related disorders, including Hashimoto's thyroiditis, Graves' disease, and thyroid nodules. The drug's therapeutic effects are attributed to its bioactive compounds, particularly flavonoids like luteolin, steroids, and terpenoids. Mechanistically, it is thought to modulate the immune system by rebalancing follicular helper T (Tfh) and follicular regulatory T (Tfr) cells, and to alleviate oxidative stress through the Nrf2/HO-1 and PI3K/Akt signaling pathways. It also shows potential in reducing vascular complications in diabetes by improving endothelial function and reducing inflammatory markers.
