Xian Tianma Jiaonang (鲜天麻胶囊) is a capsule formulation derived from fresh Gastrodia elata (Tianma), a perennial herb widely used in Traditional Chinese Medicine (TCM). Its primary active compounds include gastrodin, p-hydroxybenzyl alcohol, vanillyl alcohol, polysaccharides, and β-sitosterol. The drug exhibits neuroprotective, anti-inflammatory, antioxidant, and anticonvulsant properties. It is traditionally used for conditions such as headaches, dizziness, epilepsy, tetanus, infantile convulsions, numbness of the limbs, and rheumatic arthralgia. Modern pharmacological studies suggest its potential in treating neurodegenerative diseases like Alzheimer's and Parkinson's, improving cognitive functions, and offering cardiovascular benefits, including blood pressure reduction and enhanced myocardial oxygen supply. Its mechanism of action involves modulating various pathways, including Nrf2/HO-1 and NF-κB, enhancing GABAergic transmission, and inhibiting enzymes like β- and γ-secretase.
