Xiangfu Siwu formula is a traditional Chinese medicine (TCM) herbal decoction derived from the classic Siwu Tang (Four-Substance Decoction). It typically consists of *Cyperus rotundus* (Xiangfu) combined with the four base ingredients of Siwu Tang: *Rehmannia glutinosa*, *Angelica sinensis*, *Paeonia lactiflora*, and *Ligusticum chuanxiong*. Developed and studied by institutions such as the Nanjing University of Chinese Medicine, it is primarily used to treat primary dysmenorrhea. The formula is thought to work by regulating qi and nourishing blood, which translates in modern pharmacological terms to potential anti-inflammatory, analgesic, and antispasmodic effects on the uterine smooth muscle. It has been the subject of metabolomics studies to evaluate its intervention effects in patients with menstrual disorders.
