Xiaochaihu Granules is a traditional Chinese medicine (TCM) formulation based on the classic "Xiao Chai Hu Tang" (Minor Bupleurum Decoction) from the *Shanghan Lun*. It is composed of seven botanical ingredients: *Bupleurum chinense* (Chai Hu), *Scutellaria baicalensis* (Huang Qin), *Panax ginseng* (Ren Shen), *Pinellia ternata* (Ban Xia), *Glycyrrhiza uralensis* (Gan Cao), *Zingiber officinale* (Sheng Jiang), and *Ziziphus jujuba* (Da Zao). The formulation is traditionally used to treat "Shao Yang" syndrome, characterized by alternating chills and fever, chest discomfort, and loss of appetite. In modern clinical research, it is being evaluated for its antidepressant properties, particularly in specialized populations such as patients experiencing depression following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Its mechanism of action is multi-faceted, involving the regulation of the hypothalamic-pituitary-adrenal (HPA) axis, inhibition of neuroinflammation through the reduction of pro-inflammatory cytokines, and modulation of monoaminergic neurotransmission (serotonin and dopamine) in the central nervous system.
