Overview
Xiaoke Decoction is a Traditional Chinese Medicine (TCM) herbal formula traditionally used to treat "Xiaoke" (wasting and thirsting disorder), the TCM equivalent of diabetes mellitus. In modern clinical research, specifically in trials conducted by the Shanghai Traditional Chinese Medicine Hospital, the decoction is being evaluated for its efficacy in treating diabetic nephropathy (DN). The formula is designed to address the underlying TCM pathologies of Qi and Yin deficiency. Its therapeutic mechanism in diabetic nephropathy is primarily focused on protecting podocytes (often mistranslated as "foot cells" in registry titles) from injury, reducing glomerular basement membrane thickening, and inhibiting renal fibrosis. Pharmacological studies suggest that the decoction works by modulating inflammatory cytokines, reducing oxidative stress, and downregulating the TGF-beta signaling pathway to preserve renal function.
