Xiaoke Tongbi decoction is a Traditional Chinese Medicine (TCM) formula developed by the Guangdong Provincial Hospital of Traditional Chinese Medicine for the treatment of diabetic peripheral neuropathy (DPN). Based on the TCM therapeutic principles of invigorating Qi, nourishing Yin, and activating blood circulation to unblock collaterals, the decoction is designed to address the underlying pathology of 'Bi' syndrome (obstruction) associated with chronic diabetes (Xiaoke). The formula typically comprises a complex mixture of herbs, including *Astragalus membranaceus* (Huangqi), *Salvia miltiorrhiza* (Danshen), *Paeoniae Radix Rubra* (Chishao), and *Cassia Twig* (Guizhi). Clinical and preclinical research suggests that the decoction may exert neuroprotective effects by improving nerve conduction velocity, enhancing microcirculation, reducing oxidative stress, and inhibiting the formation of advanced glycation end-products (AGEs). It is primarily utilized as an in-hospital preparation within the Guangdong provincial healthcare system.
