Xiaoxu Ming Decoction (XXMD) is a traditional Chinese herbal formula historically used for the treatment of stroke. [1, 9] It is a combination product consisting of 12 herbs, including Ephedra sinica, Saposhnikovia divaricata, Panax ginseng, and Scutellaria baicalensis, among others. [1, 6] Clinical studies and modern research are exploring its efficacy and mechanisms in neurological conditions, primarily acute ischemic stroke and its chronic sequelae, as well as Parkinson's disease. [1, 7] The proposed mechanisms of action are multifactorial, including neuroprotective, anti-inflammatory, and anti-apoptotic effects. For ischemic stroke, XXMD is suggested to improve cerebral circulation by dilating blood vessels, inhibiting thrombosis, and protecting the blood-brain barrier. [1, 3] In preclinical models of Parkinson's disease, it has been shown to reduce inflammation, protect dopaminergic neurons, and enhance mitochondrial autophagy by upregulating proteins such as PINK1 and Parkin. [7] Research also indicates it may modulate G protein-coupled receptors (GPCRs) to regulate vascular tone. [6]
