Xiaoyu Xiezhuo Yin (also known as Xiaoyu Xiezhuo Decoction or XXD) is a Traditional Chinese Medicine (TCM) herbal formula derived from the classical 'Buyang Huanwu Decoction.' It is composed of six primary herbs: *Astragalus membranaceus*, processed *Radix cyathulae*, Peach Kernel (*Prunus persica*), Lumbricus (*Pheretima*), prepared Rhubarb (*Rheum palmatum*), and Plantain (*Plantago asiatica*). The formula is primarily investigated for its therapeutic potential in kidney-related conditions, including chronic kidney disease (CKD), acute kidney injury (AKI), and diabetic kidney disease. Its mechanism of action involves the modulation of the TGF-β1/Smad3 and HIF1 signaling pathways, regulation of the gut–kidney axis, and inhibition of the aldosterone/MR/SGK-1 signaling pathway to reduce renal interstitial fibrosis. Research is largely conducted by academic and hospital-based groups in China, such as the Zhejiang Hospital of Traditional Chinese Medicine.
