Xingnaojing

Xingnaojing injection (XNJ) is a neuroprotective agent derived from a classic Traditional Chinese Medicine (TCM) prescription known as An-Gong-Niu-Huang pill. [4] It is approved by the China Food and Drug Administration and widely used in China for the treatment of acute stroke, including both ischemic and intracerebral hemorrhage types. [1, 17] The injection is a combination of extracts from Moschus (musk), Gardenia jasminoides (gardenia fruit), Curcuma aromatica (turmeric), and Borneolum syntheticum (borneol). [1, 4] Its active components are believed to include muscone, borneol, geniposide, and curcumin. [4, 5] The proposed mechanism of action involves multiple pathways, including improving cerebral microcirculation, reducing brain edema, protecting the blood-brain barrier, and exerting anti-inflammatory and anti-apoptotic effects on neurons. [2, 6, 8] Studies suggest it can downregulate inflammatory signaling pathways like NF-kappa B and reduce levels of matrix metalloproteinases (MMPs) that are associated with stroke-related brain damage. [4, 6] It is administered via intramuscular or intravenous injection for conditions such as stroke-induced coma, traumatic brain injury, and acute alcoholism. [3, 9]
