XingpiJieyu Formula combined with escitalopram is an investigational combination therapy being studied by Peking University Sixth Hospital for the treatment of severe major depressive disorder (MDD). This treatment specifically targets patients classified with a Liver-Stagnation and Spleen-Deficiency pattern according to Traditional Chinese Medicine (TCM). The combination consists of escitalopram, a selective serotonin reuptake inhibitor (SSRI) small molecule, and XingpiJieyu Formula (XPJYF), a multi-component herbal preparation. XPJYF is intended as an adjunctive therapy to enhance the antidepressant efficacy of escitalopram and potentially reduce side effects. Its mechanism of action involves modulation of the gut microbiota-brain axis, the cAMP-PKA-CREB-BDNF signaling pathway, and mitochondrial energy metabolism. The combination is currently being evaluated in a Phase 4 clinical trial to assess its impact on clinical symptoms and biological markers such as EEG patterns and mitochondrial biosynthesis.
