Xinjia Xuanbai Chengqi Formula

Xinjia Xuanbai Chengqi Formula (新加宣白承气复方) is a modified traditional Chinese medicine (TCM) formula based on the classic "Xuanbai Chengqi Tang" (宣白承气汤). The original formula, created by Wu Jutong in the Qing Dynasty, is composed of gypsum (石膏), rhubarb (大黄), apricot kernel (杏仁), and trichosanthes peel (瓜蒌皮). It is used to clear heat from the lungs, resolve phlegm, relieve asthma, and promote bowel movement. The "Xinjia" prefix indicates an updated or adjusted version of this base formula; such modifications are common in TCM to tailor treatment for specific clinical presentations. This combination is primarily indicated for conditions characterized by high fever that does not subside, severe cough with sticky or blood-streaked sputum, shortness of breath or wheezing, sore throat, thirst, abdominal pain/distension/constipation or diarrhea—symptoms often seen in severe respiratory infections such as influenza and acute exacerbations of chronic obstructive pulmonary disease. Mechanistically it acts through clearing lung heat/inflammation ("qingre"), resolving phlegm ("huatan"), relieving asthma/wheezing ("pingchuan"), and promoting gastrointestinal motility ("tongfu").
