XTL-6865 is a combination of two fully human monoclonal antibodies (Ab68 and Ab65) that target the E2 envelope protein of the hepatitis C virus (HCV). The drug was developed to prevent hepatitis C re-infection following liver transplantation and for the treatment of chronic hepatitis C. The rationale behind combining two antibodies is to provide broad coverage against different viral quasispecies and reduce the likelihood of viral escape. Both antibodies bind to distinct epitopes on the E2 glycoprotein, neutralizing circulating virus particles. Preclinical studies demonstrated their ability to reduce viral load in animal models, while early clinical trials focused on safety, pharmacokinetics, and preliminary antiviral activity in humans[1][2][3][7][9].
