Xuanbai Chengqi Decoction is a classical traditional Chinese medicine (TCM) prescription originally documented in the Qing dynasty text "Systematic Differentiation of Warm Diseases" (Wen Bing Tiao Bian). The canonical formula consists of four main herbal ingredients: gypsum (Shi Gao), rhubarb (Da Huang), bitter apricot seed (Xing Ren), and snakegourd peel (Gua Lou Pi)[2][7]. In some modern clinical studies, modified versions may include additional herbs such as immature bitter orange, perilla seed, descurainia seed, Thunberg fritillary bulb, stemona root, loquat leaf, and red tangerine peel[2]. The decoction is primarily used to clear lung heat and phlegm, relieve asthma symptoms by purging heat from the lungs and intestines, promote bowel movements, and reduce inflammation[3][7]. Pharmacological research indicates that it downregulates inflammatory mediators such as IL-6 and TNF-α to mitigate lung inflammation. It also modulates immune responses to pathogens[2][4], inhibits NLRP3 inflammasome activation involved in pyroptosis during acute lung injury[4], strengthens intestinal barrier function via rhubarb components[5], balances Th17/Treg cells in COPD models[6], and regulates multiple pathways relevant to viral infection and inflammatory response according to network pharmacology studies for COVID-19 treatment[8]. It has been recommended by Chinese health authorities for conditions including acute exacerbations of chronic obstructive pulmonary disease (AECOPD) with phlegm-heat syndrome[1], acute lung injury/ARDS[4][7], severe pneumonia including viral etiologies like influenza A or SARS-CoV-2/COVID-19[5][8], pulmonary fibrosis,[7] monkeypox-related respiratory symptoms,[7] as well as general use for coughs with excessive phlegm or wheezing.
