Xuanfei Baidu is a traditional Chinese medicine (TCM) formula developed specifically for treating COVID-19. It was designed by Academician Zhang Boli and Professor Liu Qingquan during the COVID-19 pandemic in China. The formula is available in different forms, including decoction (XFBD) and granule formulations. Xuanfei Baidu consists of 13 Chinese medicinal herbs: Ephedrae Herba (Mahuang), Gypsum Fibrosum (Shigao), Atractylodis Rhizoma (Cangzhu) (stir-baked with bran), Pogostemonis Herba (Guanghuoxiang), Artemisiae Annuae Herba (Qinghao), Polygoni Cuspidati Rhizoma et Radix (Huzhang), Verbenae Herba (Mabiancao), Coix Seed (Yiyiren), Phragmitis Rhizoma (Lugen), Descurainiae Semen Lepidii Semen (Tinglizi), Armeniacae Semen Amarum (Kuxingren), Citri Grandis Exocarpium (Huajuhong), Glycyrrhizae Radix et Rhizoma (Gancao). Xuanfei Baidu is recognized as one of the "three medicines and three prescriptions" recommended for COVID-19 treatment in China's national diagnosis and treatment guidelines. It has shown efficacy in shortening the duration from positive to negative SARS-CoV-2 nucleic acid tests, reducing hospitalization duration, improving clinical symptoms including fever, fatigue, cough, and appetite, preventing progression from moderate to severe disease states, and increasing white blood cell and lymphocyte counts. Research indicates that Xuanfei Baidu works through multiple mechanisms: anti-inflammatory effects (regulating macrophage-mediated immune inflammation, reducing NLRP3 inflammasome activation, inhibiting IL-6/STAT3 signaling pathway), mitochondrial dynamics regulation, and host metabolism modulation. According to traditional Chinese medicine theory, Xuanfei Baidu has the efficacy of "promoting the lung and resolving dampness, clearing heat and removing pathogeneses, purging the lung and depoisoning". It is specifically indicated for the TCM syndrome of "damp-toxin constraint in the lungs". Clinical studies have reported good safety with no serious adverse events.
