Yijian04 (also known as 疫建04) is an investigational immunomodulatory therapy consisting of placenta-derived heat shock protein gp96 (also known as GRP94 or HSP90B1) injection, developed by Foshan Rexiu Biotechnology (Foshan Heat Shock Biotech). Gp96 is a major endoplasmic reticulum chaperone that plays a critical role in maintaining immune homeostasis and regulating regulatory T cell (Treg) stability and function. In amyotrophic lateral sclerosis (ALS / motor neuron disease), chronic neuroinflammation driven by immune dysregulation accelerates motor neuron degeneration. Yijian04 acts by binding to receptors such as Toll-like receptor 2 (TLR2), Toll-like receptor 4 (TLR4), and CD91 (LRP1) on immune cells, which promotes the activation and expansion of Tregs (including activated Tregs) and suppresses inflammatory CD14+ monocytes, natural killer (NK) cells, and effector T cells. This restores immune homeostasis, suppresses chronic neuroinflammation, and protects motor neurons from inflammatory damage. Yijian04 is currently in clinical development in China for the treatment of ALS, showing promising safety, tolerability, and preliminary efficacy in slowing disease progression.
