Yiqi Juanshen decoction (YQJSD) is a **traditional Chinese medicine (TCM) formulation** used for the treatment of **chronic kidney disease (CKD)** and specifically targeting **renal interstitial fibrosis (RIF)**. The formulation consists of multiple herbal components, with identified active compounds including terpenoids, flavonoids, phenylpropanoids, and alkaloids. Key bioactive components include Kaempferide, Hydroxygenkwanin, Demethoxycurcumin, Apigenin, and 1,7-bis(4-hydroxyphenyl) heptane-3-one. YQJSD works through multiple mechanisms: it **inhibits the NF-κB signaling pathway** to reduce pro-inflammatory cytokines (IL-1β, TNF-α, CCR1), **suppresses epithelial-mesenchymal transition (EMT)** by modulating Snail1, Vimentin, Twist1, FSP1, and E-cadherin expression, and **regulates the LOXL2/PI3K/AKT signaling pathway** to reduce fibrosis. The decoction effectively reduces serum creatinine and blood urea nitrogen levels, decreases extracellular matrix deposition (COL4α1, Fibronectin), and downregulates fibrosis markers like α-SMA. In preclinical studies using the unilateral ureteral obstruction (UUO) mouse model, YQJSD demonstrated dose-dependent therapeutic effects with no hepatorenal toxicity at high doses.
