Yisui Granule (YSG) is a Traditional Chinese Medicine (TCM) formulation developed primarily for the treatment of myelodysplastic syndromes (MDS), particularly low and medium-risk categories. Its therapeutic mechanism is characterized by epigenetic modulation, specifically acting as a demethylating agent. Research indicates that YSG reduces the expression of DNA methyltransferase 1 (DNMT1), which leads to the demethylation and subsequent reactivation of the secreted frizzled-related protein 5 (sFRP5) gene. Reactivation of sFRP5 functions as an antagonist to the Wnt/β-catenin signaling pathway, effectively down-regulating oncogenic components such as Wnt3a, β-catenin, c-Myc, and cyclinD1. This suppression of abnormal signaling pathways contributes to anti-tumor effects and improved hematopoiesis, offering a potential alternative or adjunctive therapy to standard hypomethylating agents like decitabine.
