Yiyi Fuzi Baijiang Powder is a classic traditional Chinese medicine (TCM) formula composed of three herbs: Coix seed (Yi Yi Ren/Semen Coicis), processed Aconite root (Fu Zi/Radix Aconiti Lateralis Preparata), and Patrinia herb (Bai Jiang Cao/Herba Patriniae)[2][5]. It was first recorded in the "Essentials from the Golden Cabinet" (~220 AD) and is traditionally used to clear internal abscesses, expel pus, reduce swelling, warm Yang energy, and disperse phlegm[2][3][5]. In modern clinical practice and research, it is most commonly used for conditions such as colorectal cancer[1][7], ulcerative colitis[4][6], appendicitis with abscess formation[5], pelvic inflammatory disease[2], skin infections with pus formation, gynecological inflammation, chronic inflammatory diseases of the intestine including ulcerative colitis and chronic appendicitis. \nMechanistically, network pharmacology studies suggest that its anti-tumor and anti-inflammatory effects are mediated by downregulation of SMOX expression via anti-inflammatory actions; regulation of TLR4/NF-κB signaling pathway; inhibition of NLRP3 inflammasome activation; reduction in pro-inflammatory cytokines such as IL-1β and TNF-α; repair of intestinal epithelial barrier function; modulation of intestinal microbiota composition; regulation of cell cycle/apoptosis/inflammatory pathways through multiple targets including progesterone receptor, prostaglandin G/H synthase 1/2 (COX-1/COX-2), gamma-aminobutyric acid receptor subunit alpha-1 (GABRA1), beta-2 adrenergic receptor (ADRB2), sodium channel protein type 5 subunit alpha among others[1][4][6][7]. \nThe formula’s main active components include flavonoids like luteolin (LUT), sitosterol (SIT), myricetin aldehyde (MYA), vanillic acid (VAN)[1]; coix seed oil constituents are also prominent bioactives[9].
