Young TIL (Tumor-Infiltrating Lymphocytes) is an autologous adoptive cell therapy developed by the National Cancer Institute (NCI) Surgery Branch. The "young" designation refers to a modified manufacturing process where TILs are expanded ex vivo for a shorter duration (typically 2-3 weeks) compared to traditional TIL products. This abbreviated culture period is designed to prevent terminal differentiation and T-cell exhaustion, thereby preserving the cells' proliferative capacity, longevity, and effector function upon re-infusion. In clinical applications for non-small cell lung cancer (NSCLC), the patient's own tumor-resident lymphocytes are harvested from resected tumor tissue, expanded with high-dose interleukin-2 (IL-2), and administered following a non-myeloablative lymphodepleting conditioning regimen (typically cyclophosphamide and fludarabine) to facilitate robust anti-tumor immune responses.
