Yttrium (90Y) edotreotide (commonly known as 90Y-DOTATOC or Onalta) is an investigational radiolabeled somatostatin analogue used in Peptide Receptor Radionuclide Therapy (PRRT). It consists of the somatostatin receptor ligand edotreotide (DOTATOC) chelated with the beta-emitting radioisotope yttrium-90. The drug selectively targets somatostatin receptors (primarily SSTR2 and SSTR5) overexpressed on the surface of neuroendocrine tumors (NETs) and other somatostatin receptor-positive malignancies. Upon binding, the radiopharmaceutical is internalized, delivering targeted, high-energy beta radiation directly to the tumor cells to induce DNA damage and cell death while minimizing damage to surrounding healthy tissues. Originally developed by Novartis and later licensed to Molecular Insight Pharmaceuticals (acquired by Progenics Pharmaceuticals, now Lantheus), the drug has been evaluated in clinical trials for neuroendocrine tumors, meningioma, thyroid cancer, and lung cancer.
