Overview

Yttrium-86 is a positron-emitting radioisotope of yttrium with a half-life of approximately 14.74 hours, making it suitable for Positron Emission Tomography (PET) imaging. It is primarily utilized as a diagnostic surrogate for the therapeutic isotope yttrium-90 (Y-90). Because yttrium-86 and yttrium-90 are chemically identical, Y-86 labeled compounds (such as monoclonal antibodies or peptides) can be used to accurately predict the biodistribution and radiation dosimetry of Y-90 therapies, a concept central to the field of theranostics. It is typically produced in a cyclotron via the (p,n) nuclear reaction on enriched strontium-86 targets. Clinical applications often involve its conjugation to targeting vectors like DOTATOC or various antibodies to visualize target expression and estimate therapeutic window before administering the higher-energy beta-emitting Y-90 counterpart.
