**Yttrium-90-DOTA-BC8** is a radiolabeled monoclonal antibody that targets the CD45 antigen, which is ubiquitously expressed on most hematopoietic cells. The molecule consists of the murine monoclonal antibody BC8 (directed against human CD45), conjugated to DOTA (a chelator), which in turn holds the radioactive isotope yttrium-90 (a beta-emitter). The drug is designed for targeted radioimmunotherapy, delivering cytotoxic radiation directly to hematologic cancer cells—including acute myeloid leukemia, myelodysplastic syndrome, acute lymphoblastic leukemia, lymphoma, and multiple myeloma—while sparing normal tissues. It has been studied as a conditioning regimen prior to allogeneic hematopoietic stem cell transplantation in these diseases, sometimes in combination with agents such as fludarabine and total-body irradiation. The radiolabeled antibody is used to ablate malignant and normal hematopoietic cells to facilitate donor cell engraftment and reduce disease burden[1][2][4][5][7][10].
