yttrium-90 DOTA-biotin-avidin ([90Y]DBA) is an experimental radiopharmaceutical developed for the intravesical treatment of non-muscle invasive bladder cancer (NMIBC). It consists of the high-energy beta-emitting radioisotope yttrium-90 chelated by DOTA and conjugated to a biotin-avidin complex. The mechanism of action involves the delivery of localized, short-range beta radiation directly to the bladder urothelium to induce DNA damage and cell death in malignant tissues. A critical design feature of the DBA complex is its high molecular weight, which prevents systemic absorption through the bladder wall into the bloodstream. This localization maximizes the therapeutic effect on superficial tumors while minimizing systemic toxicity and preserving the integrity of the underlying bladder histology. Preclinical studies in orthotopic murine models have demonstrated significant antitumor activity and a favorable safety profile.
