Yttrium-90 ferric hydroxide macroaggregate is a radiopharmaceutical agent primarily utilized in radiation synovectomy (synoviorthesis) for the treatment of chronic inflammatory joint diseases. It consists of the high-energy beta-emitting radioisotope yttrium-90 incorporated into ferric hydroxide particles, typically ranging in size from 10 to 100 micrometers. This macroaggregate formulation is designed for intra-articular injection; its large particle size ensures retention within the synovial space, delivering concentrated radiation to the inflamed synovial lining while minimizing leakage to the lymphatic system. The beta radiation, which has a mean penetration depth of approximately 3.6 mm, induces localized tissue necrosis and fibrosis of the hyperplastic synovium, effectively reducing joint effusion, swelling, and pain. Historically, similar colloidal or aggregate forms of yttrium-90 have also been explored for the intraperitoneal treatment of malignant effusions in cancers such as ovarian cancer.
