Yttrium-90 hydroxyapatite is a radiopharmaceutical used for radiosynovectomy—the intra-articular injection of radioactive material to treat chronic synovitis and inflammatory joint diseases. The drug consists of the beta-emitting radionuclide yttrium-90 bound to particles of hydroxyapatite (particle size typically 1–10 μm). When injected into the joint space, it delivers localized radiation to the synovial membrane, leading to reduction in inflammation and synovial proliferation. It is primarily indicated for conditions such as rheumatoid arthritis and hemophilic arthropathy affecting large joints (e.g., knee), as well as pigmented villonodular synovitis. The mechanism involves beta radiation-induced ablation of inflamed or hypertrophic synovial tissue while sparing surrounding structures due to limited tissue penetration[1][4][7][9]. The preparation is typically compounded at hospital radiopharmacies using cold kits containing sterile hydroxyapatite particles that are labeled with carrier-free yttrium-90 chloride solution just prior to administration[9]. Clinical studies have shown significant improvement in joint function and reduction in bleeding episodes after treatment[1][4].
