This is a multi-modality antineoplastic regimen combining intrahepatic arterial radioembolization using yttrium-90 microspheres with systemic chemotherapy FOLFOX6 (folinic acid/leucovorin, fluorouracil, oxaliplatin) and the anti-angiogenic monoclonal antibody bevacizumab, most commonly investigated for liver-dominant metastatic colorectal cancer. Yttrium-90 microspheres are delivered via selective hepatic arterial catheterization to lodge within tumor arterioles and emit high-energy beta radiation, achieving localized tumoricidal dosing with minimal embolic effect. FOLFOX6 provides cytotoxic chemotherapy via DNA synthesis inhibition and DNA crosslinking, while bevacizumab neutralizes VEGF-A to inhibit tumor angiogenesis and can normalize tumor vasculature. This combination has been studied to improve control of liver metastases and convert to resection in unresectable colorectal liver metastases by pairing local radiotherapy with systemic therapy and anti-angiogenesis.
