Yttrium-90 resin microspheres + cisplatin + gemcitabine is a combination therapeutic regimen investigated for the treatment of unresectable intrahepatic cholangiocarcinoma (ICC). This sequential therapy involves selective internal radiation therapy (SIRT) using yttrium-90 (Y-90) resin microspheres, followed by systemic chemotherapy with cisplatin and gemcitabine. The Y-90 resin microspheres are radioactive particles delivered via the hepatic artery to provide localized beta-radiation directly to liver tumors, a process known as radioembolization. Cisplatin is a platinum-based antineoplastic agent that induces DNA cross-linking, while gemcitabine is a nucleoside analog that inhibits DNA synthesis. This combination approach aims to provide potent local tumor control through radiation while addressing systemic disease through chemotherapy. Clinical evaluation in phase 2 trials has demonstrated promising overall survival outcomes in patients with advanced ICC.
