Yunpi Tiaozhi Formula (YPTZF) is a Traditional Chinese Medicine (TCM) herbal compound developed by the Shanghai Tenth People's Hospital for the treatment of type 2 diabetes mellitus (T2DM) and associated macrovascular complications, specifically carotid atherosclerosis. The formula is designed based on the TCM principles of fortifying the spleen (Yunpi) to resolve dampness and regulating lipids (Tiaozhi) to clear metabolic turbidity. It typically comprises a standardized mixture of herbs including *Astragalus membranaceus*, *Atractylodes macrocephala*, and *Coptis chinensis*. Mechanistically, YPTZF is thought to improve insulin sensitivity and optimize lipid profiles by modulating the gut microbiota composition and activating the PI3K/Akt signaling pathway. These actions contribute to reduced systemic inflammation and improved endothelial function, which are critical for slowing the progression of carotid intima-media thickness (CIMT) in diabetic patients.
