Z-BEAM + ASCT is an intensified conditioning regimen used prior to autologous stem cell transplantation (ASCT) for patients with relapsed or poor-risk B-cell non-Hodgkin lymphoma. The regimen incorporates the radioimmunotherapy agent Zevalin (yttrium Y 90 ibritumomab tiuxetan), which targets the CD20 antigen on B-cells to deliver localized beta radiation, alongside the standard BEAM chemotherapy (carmustine, etoposide, cytarabine, and melphalan). Rituximab is also typically administered as part of the protocol. This combination aims to enhance the cytoreductive effect on malignant B-cells through multiple mechanisms, including targeted radiation, antibody-dependent cellular cytotoxicity, and DNA damage from alkylating agents and antimetabolites, thereby improving outcomes in aggressive or refractory lymphomas.
