Zabilugene almadenorepvec (VCN-01) is an oncolytic adenovirus therapy designed to selectively replicate within and lyse tumor cells, particularly in solid tumors. The virus is engineered to exploit defects in the retinoblastoma (RB) pathway commonly found in cancer cells, allowing for selective replication and subsequent tumor cell destruction. In addition to direct oncolysis, the therapy induces immunogenic cell death and stimulates anti-tumor immune responses by releasing tumor antigens into the microenvironment. Zabilugene almadenorepvec has been evaluated primarily for metastatic pancreatic ductal adenocarcinoma (mPDAC), where it is administered intravenously prior to standard-of-care chemotherapy with gemcitabine/nab-paclitaxel. Clinical trials have demonstrated increased overall survival, progression-free survival, and duration of response when combined with chemotherapy compared to chemotherapy alone. The drug has shown a favorable safety profile with transient and reversible adverse events[6][9].
