ZR2 + mini-CHOP is an experimental chemo-immunotherapy regimen being investigated for the treatment of elderly patients with newly diagnosed diffuse large B-cell lymphoma (DLBCL). Developed by The First Affiliated Hospital of Zhejiang University Medical College, the regimen combines the 'ZR2' triplet—consisting of the next-generation Bruton's tyrosine kinase (BTK) inhibitor zanubrutinib, the anti-CD20 monoclonal antibody rituximab, and the immunomodulatory agent lenalidomide—with a reduced-dose CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) backbone. This combination aims to improve efficacy in elderly populations who may not tolerate full-dose R-CHOP, by leveraging the synergistic effects of targeted therapy and immunomodulation alongside traditional cytotoxic chemotherapy.
