Overview

This sequential combination regimen consists of a chemo-free induction phase (ZR2) followed by standard RCHOP immunochemotherapy. The induction phase combines zanubrutinib (a selective BTK inhibitor), rituximab (an anti-CD20 monoclonal antibody), and lenalidomide (an immunomodulatory agent). This is followed by the RCHOP regimen, which typically includes rituximab, cyclophosphamide, an anthracycline (such as epirubicin or doxorubicin), a vinca alkaloid (such as vindesine or vincristine), and prednisone. The regimen is specifically being evaluated in Phase II clinical trials led by Zhejiang Cancer Hospital for elderly patients (aged 75 and older) with newly diagnosed diffuse large B-cell lymphoma (DLBCL). The strategy aims to provide a potent initial response using targeted agents and immunomodulators to reduce the total chemotherapy burden and improve tolerability in a population that often cannot handle standard-intensity chemotherapy.
