ZCR + CHOP

ZCR + CHOP is a combination chemotherapy regimen currently under investigation for the treatment of diffuse large B-cell lymphoma (DLBCL), specifically in patients with treatment-naive double-expressor DLBCL (DE-DLBCL). DE-DLBCL is characterized by the co-expression of MYC (≥40%) and BCL2 (≥50%) as determined by immunohistochemistry. The regimen combines the ZCR protocol (Zanubrutinib, Chidamide, and Rituximab) with the standard CHOP chemotherapy regimen (Cyclophosphamide, Doxorubicin Hydrochloride, Vincristine Sulfate, and Prednisone). It is being studied in a multicenter, open-label, single-arm phase 2 clinical trial (NCT05527912) in China. The treatment protocol in the trial is adaptive: * If patients achieve a complete response (CR) after cycle 2, treatment continues with ZCR alone for up to 8 total cycles. * If complete response is not achieved after cycle 2, ZCR-CHOP is administered for an additional 6 cycles. Early results from the trial suggest that ZCR±CHOP (referring to the adaptive strategy) may be a promising regimen with active antitumor effects and acceptable toxicity in treatment-naive DE-DLBCL patients.
