Zemiva is a radiopharmaceutical molecular imaging agent, chemically known as iodofiltic acid I 123, developed for single-photon emission computed tomography (SPECT) imaging to rapidly detect cardiac ischemia in patients with suspected acute coronary syndromes. It is a radioiodinated fatty acid analog that is preferentially taken up and metabolized by normally perfused myocardium, while ischemic myocardial tissue, which switches from fatty acid to carbohydrate metabolism, shows reduced tracer uptake, creating a perfusion–metabolism mismatch that facilitates detection of recent or ongoing ischemia up to roughly a day after an ischemic event.[7][8][14] Molecular Insight Pharmaceuticals advanced Zemiva through at least Phase 2b clinical trials as its lead cardiology imaging candidate for the diagnosis of cardiac ischemia, positioning it as a targeted SPECT tracer with potentially greater sensitivity and extended ischemia detection windows compared with conventional perfusion tracers.[6][7][8][14][16]
