Zevorcabtagene autoleucel is a fully human, autologous chimeric antigen receptor (CAR) T-cell therapy that targets B cell maturation antigen (BCMA), which is highly expressed on malignant plasma cells in multiple myeloma. The product is generated by transducing patient-derived (autologous) T cells with a lentiviral vector encoding a CAR construct composed of a fully human BCMA-specific single-chain variable fragment (scFv), CD8α hinge and transmembrane domains, 4‑1BB costimulatory domain, and CD3ζ activation domain. Upon infusion, these engineered T cells recognize and selectively eliminate BCMA-expressing tumor cells through immunologic cytotoxicity. Zevorcabtagene autoleucel was developed by CARsgen for the treatment of relapsed or refractory multiple myeloma in patients who have progressed after at least three prior lines of therapy including a proteasome inhibitor and an immunomodulatory agent. It received its first approval in China in February 2024 for this indication; clinical development continues internationally[1][3][4][5].
