ZeXieYin formula (ZXYF) is a Traditional Chinese Medicine (TCM) decoction composed of *Alisma plantago-aquatica L.* (Zexie), *Atractylodis macrocephala Koidz.* (Baizhu), and *Pyrola calliantha Andres.* (Luxiancao) in a 2:2:1 ratio. Originally documented in the classical text *HuangDiNeiJin*, ZXYF is primarily investigated for its therapeutic potential in treating atherosclerosis and hyperlipidemia. Its mechanism of action involves the modulation of the farnesoid X receptor (FXR)/fibroblast growth factor 15 (FGF15) signaling axis, which regulates enterohepatic bile acid circulation and promotes de novo bile acid synthesis via the upregulation of CYP7A1. Additionally, ZXYF enhances intestinal barrier integrity by increasing the expression of tight junction proteins such as ZO-1 and Claudin-1, reshapes the gut microbiota (specifically increasing the abundance of *Akkermansia muciniphila*), and facilitates biliary cholesterol excretion through the activation of ABCG5 and ABCG8 transporters.
