Overview
Zhenwu decoction (Zhenwu Tang) is a classic Traditional Chinese Medicine (TCM) formula first recorded in the *Shanghan Lun* (Treatise on Febrile Diseases). It is traditionally indicated for Yang deficiency of the Spleen and Kidney, which manifests as fluid retention, edema, and urinary difficulty. The formula typically consists of five herbal components: *Poria cocos* (Fuling), *Paeonia lactiflora* (Baishao), *Zingiber officinale* (Shengjiang), *Atractylodes macrocephala* (Baizhu), and processed *Aconitum carmichaelii* (Fuzi). In modern clinical research, Zhenwu decoction is investigated for its diuretic and cardioprotective properties, particularly in the treatment of refractory cirrhotic ascites, chronic heart failure, and chronic kidney disease. Its mechanism of action involves the regulation of the renin-angiotensin-aldosterone system (RAAS), modulation of aquaporin expression (specifically AQP2), and improvement of renal hemodynamics to promote water metabolism.
