Zhenwu Tang (also known as Zhenwu Decoction or True Warrior Decoction) is a classic Traditional Chinese Medicine (TCM) herbal formula first documented in the *Shanghan Lun* (Treatise on Febrile Diseases). It is traditionally indicated for Yang deficiency of the Spleen and Kidney characterized by internal retention of water and dampness, manifesting as edema, palpitations, and dizziness. In modern clinical research, Zhenwu Tang is primarily investigated for the treatment of chronic heart failure, particularly heart failure with reduced ejection fraction (HFrEF). The formula is a combination of five botanical ingredients: *Aconitum carmichaelii* (Fuzi), *Poria cocos* (Fuling), *Atractylodes macrocephala* (Baizhu), *Paeonia lactiflora* (Baishao), and *Zingiber officinale* (Shengjiang). Its therapeutic effects are attributed to its ability to improve cardiac hemodynamics, inhibit the overactivation of the renin-angiotensin-aldosterone system (RAAS), reduce myocardial fibrosis, and modulate water-electrolyte balance through the regulation of aquaporins.
