Zhilong Huoxue Tongyu Capsule is a traditional Chinese medicine (TCM) oral capsule formulation used primarily for promoting blood circulation, removing blood stasis, and unblocking meridians. It is indicated for the recovery period of ischemic stroke (arteriosclerotic cerebral infarction) in patients diagnosed with Qi deficiency and blood stasis syndrome according to TCM principles. Clinical symptoms include hemiplegia, numbness on one side of the body, facial paralysis, and speech difficulties[1][7]. The formula contains multiple herbal ingredients such as Astragalus membranaceus (Huangqi), Whitmania pigra (Shuizhi/leech), Ligusticum chuanxiong (Chuanxiong), Angelica sinensis (Danggui), Carthamus tinctorius (Honghua/safflower), Prunus persica seed (Taoren/peach kernel), Paeonia veitchii root (Chishao/red peony root), Aucklandia lappa root (Muxiang/costus root), Acorus tatarinowii rhizome (Shichangpu/sweet flag rhizome), Pheretima aspergillum earthworm extract (Dilong/earthworm extract), Taxillus chinensis stem with leafs parasitic on mulberry tree branchlets or leaves(Sangjisheng/mulberry mistletoe) and Acanthopanax senticosus extractum(Eleutherococcus senticosus)[7]. Pharmacological studies suggest that the capsule exerts neuroprotective effects in animal models of intracerebral hemorrhage by reducing brain injury and inflammation. Mechanistically, it downregulates pro-inflammatory cytokines such as interleukin-1β and tumor necrosis factor-alpha at both protein and mRNA levels[6][3], modulates long non-coding RNAs involved in neural injury response[3][6], inhibits nuclear factor kappa B signaling pathway activation[4], suppresses NLRP3 inflammasome expression[4], regulates oxidative stress responses, promotes angiogenesis via vascular endothelial growth factor A pathways, influences apoptosis-related genes/proteins such as MMP9/NOS3/L6/TNF/CCL2/VEGFA through multi-target actions involving AGE-RAGE/HIF-1/Apelin signaling pathways[5].
