Zhuanggu Guanjie Pill

Zhuanggu Guanjie Pill (ZGGJP) is a modern Chinese Patent Medicine (CPM) formulation consisting of 12 herbal ingredients, including *Herba Epimedii*, *Radix Rehmanniae Preparata*, and *Psoraleae Fructus*. It has been used in China for over 30 years primarily for the treatment of osteoporosis and degenerative joint diseases such as osteoarthritis, specifically those associated with "liver and kidney deficiency" in Traditional Chinese Medicine. The drug exerts its therapeutic effects by inhibiting bone resorption through the suppression of osteoclast differentiation, reduction of tartrate-resistant acid phosphatase (TRAP) activity, and downregulation of matrix metalloproteinase-9 (MMP-9) expression. Additionally, it provides chondroprotective effects by antagonizing cytokine-induced chondrocyte apoptosis. Clinically, ZGGJP is noted for its potential to cause drug-induced liver injury (DILI), which is the subject of ongoing surveillance studies. It is also a known inducer of several cytochrome P450 (CYP) enzymes, including CYP1A2, CYP2B6, CYP2C9, and CYP3A, which may lead to significant herb-drug interactions.
