INCB000928 (zilurgisertib) is a potent, orally bioavailable, and highly selective small molecule inhibitor of activin receptor-like kinase 2 (ALK2). It is under clinical evaluation for the treatment of anemia due to myeloproliferative disorders such as myelofibrosis and polycythemia vera, both as monotherapy and in combination with ruxolitinib. The drug acts by inhibiting ALK2-mediated hepcidin production; elevated hepcidin levels are associated with anemia in these conditions. By reducing hepcidin, INCB000928 may improve iron availability and alleviate anemia.
