Overview

89Zr-avelumab is an investigational radiopharmaceutical designed for immuno-positron emission tomography (immuno-PET) imaging. It consists of the monoclonal antibody avelumab, which targets programmed death-ligand 1 (PD-L1), conjugated to the positron-emitting radioisotope zirconium-89 (89Zr), typically using a chelator such as desferrioxamine (DFO). As a diagnostic imaging agent, it is used to non-invasively visualize and quantify PD-L1 expression in both primary and metastatic tumor lesions. This allows for the assessment of target accessibility and the heterogeneity of PD-L1 expression across different tumor sites in vivo, which may overcome the limitations of traditional biopsy-based immunohistochemistry. Clinically, it is being evaluated as a biomarker to predict patient response to avelumab and other PD-1/PD-L1 checkpoint inhibitor therapies in cancers such as non-small cell lung cancer (NSCLC).
