Zirconium-89 patritumab deruxtecan is a radiolabeled antibody-drug conjugate (ADC) designed for positron emission tomography (PET) imaging. It consists of the HER3-targeting ADC patritumab deruxtecan (HER3-DXd) labeled with the positron-emitting radioisotope zirconium-89 (89Zr). Patritumab deruxtecan itself comprises a human IgG1 monoclonal antibody (patritumab) linked to a topoisomerase I inhibitor payload (deruxtecan) via a cleavable tetrapeptide-based linker. By utilizing the 89Zr label, this agent allows for the non-invasive quantification and visualization of HER3 expression and drug distribution in vivo. It is primarily being investigated in clinical trials for patients with advanced non-small cell lung cancer (NSCLC), particularly those with EGFR mutations, to optimize dosing and predict treatment response.
