Overview
Zishen Zhuanggu is a Traditional Chinese Medicine (TCM) formulation developed by Nanjing University of Chinese Medicine, specifically designed for the treatment of primary osteoporosis. The formula is based on the TCM therapeutic principle of tonifying the kidney and strengthening the bone (Zishen Zhuanggu). It typically consists of a combination of herbal ingredients, including *Epimedium brevicornu*, *Rehmannia glutinosa*, and *Drynaria fortunei*. Mechanistically, Zishen Zhuanggu regulates bone metabolism by modulating the OPG/RANKL/RANK signaling pathway; it increases the expression of Osteoprotegerin (OPG) and Bone Morphogenetic Protein 2 (BMP2) while inhibiting the expression of Receptor Activator of Nuclear Factor Kappa-B Ligand (RANKL). This dual action promotes osteoblast differentiation and bone formation while suppressing osteoclast activity and bone resorption. It is primarily administered orally in capsule or granule form and has been evaluated in Phase 2 clinical trials in China.
