Overview
Zmax is a single-dose, extended-release microsphere formulation of the macrolide antibiotic azithromycin, developed by Pfizer. It is specifically indicated for the treatment of acute otitis media in children and community-acquired pneumonia or acute bacterial sinusitis in adults. Utilizing a sustained-release technology, the formulation is designed to deliver a full therapeutic course (60 mg/kg for pediatric otitis media) in a single oral administration, thereby maximizing patient compliance and delivering the highest drug concentration when the bacterial burden is greatest. Azithromycin exerts its antibacterial effect by binding to the 50S ribosomal subunit of susceptible microorganisms, which inhibits RNA-dependent protein synthesis. It is effective against common respiratory pathogens, including *Streptococcus pneumoniae*, *Haemophilus influenzae*, and *Moraxella catarrhalis*.
