Overview

The 1:1:2 transfusion ratio is a clinical resuscitation protocol used primarily in the management of massive hemorrhage and traumatic injury (StatPearls, 2023). Unlike traditional pharmacological agents that interact with specific molecular receptors, this protocol dictates the proportional administration of blood products—specifically one unit of fresh frozen plasma and one unit of platelets for every two units of packed red blood cells (Holcomb et al., 2015). This strategy is a core component of damage control resuscitation, designed to mitigate the "lethal triad" of acidosis, hypothermia, and coagulopathy. By providing clotting factors and platelets alongside oxygen-carrying red cells, the protocol attempts to stabilize the patient's physiological state until definitive surgical or interventional hemostasis can be achieved. Clinical trials, such as the PROPPR trial, have evaluated various ratios to determine the optimal balance for improving survival in exsanguinating patients (JAMA, 2015). The protocol is essential for preventing trauma-induced coagulopathy, which often results from the dilution of endogenous factors during large-volume crystalloid resuscitation.

Mechanism of action
