Absorbable gelatin sponge, widely recognized by the brand name Gelfoam, is a sterile, water-insoluble, and porous surgical device derived from purified porcine skin gelatin (FDA, 2023). It is primarily utilized as a topical hemostatic agent to control bleeding in surgical sites where conventional methods like ligation or cautery are ineffective or impractical (StatPearls, 2023). The sponge functions by providing a physical matrix that absorbs blood, leading to the entrapment and activation of platelets and the initiation of the intrinsic coagulation pathway (Pfizer, 2021). This mechanical framework facilitates the formation of a stable fibrin clot within the sponge's interstices. While the gelatin matrix itself is pharmacologically inactive, it is often used in conjunction with topical thrombin to enhance efficacy (PubMed, 2022). The material is fully absorbable, typically being digested by tissue enzymes and phagocytosed within four to six weeks without causing significant inflammatory responses (NIH, 2023). The phrase "blood components within Gelfoam matrix" specifically describes the physiological state where platelets and clotting factors are concentrated within the device to achieve hemostasis. Clinical safety concerns include the risk of infection if left in contaminated sites and the potential for the sponge to expand and compress adjacent structures, such as nerves (FDA, 2023).
