Overview

The acid-base equilibrium is regulated by the bicarbonate buffer system. Alkalinization via conversion of lactate to bicarbonate is a physiological process where lactate is metabolized, primarily by the liver, into bicarbonate ions. This helps correct metabolic acidosis. Lactate, commonly administered as sodium lactate in solutions like Lactated Ringer's, is converted to pyruvate and then metabolized to produce carbon dioxide, water, and new bicarbonate. This process depends on intact liver function and sufficient tissue perfusion.

Mechanism of action

Lactate is metabolized into pyruvate, then enters gluconeogenesis or oxidative metabolism, producing carbon dioxide and water. This process generates new bicarbonate ions, increasing plasma bicarbonate concentration and counteracting acidosis.
